If you are scheduled for surgery click the tab "Patients" above and in the section "Patient Forms" download and complete the necessary pre-admission patient forms.
Before Surgery
Your physician will discuss your operation with you. Be sure to tell your doctor about any medications you are taking and ask whether or not you should take them the day of surgery.
You MUST make arrangements for someone to drive you home following surgery and have someone stay with you for the first 24 hours following surgery.
A parent or legal guardian must accompany patients under the age of 18. It is extremely important that you do not eat or drink ANYTHING after midnight the night before your surgery unless your surgeon or anesthesiologist instructs otherwise.
Surgery for Children
Children are encouraged to bring a favorite toy, stuffed animal or blanket with them the day of surgery. Parents are required to stay with the child while he or she is in the preoperative and postoperative areas and will be allowed to accompany the child into the operating room while the child goes to sleep. It is important that the parent(s) remain in the building while the surgery is in progress.
Note: While we pay special attention to our younger patients, it is no substitute for a parent's presence. The comfort and care you provide will make all the difference in your child’s experience.
